Documents needed for Sanitas NLV health insurance

Very little is needed to get a quote. More is needed to complete an application — and the health questions belong only to that second stage. Below, the two stages are set out separately so you know what belongs where.

Two Different Stages

What each stage actually asks for.

The commonest mistake is preparing for the wrong one. A quote needs information; an application needs information and documents.

About the application stage. What follows — what it asks for, and how identity details are handled — describes the application process. The instructions supplied with your application are the authority for your own case, and we will tell you exactly what is needed for your own application.

StageWhat it asks for
The quoteYour ages, intended start date, destination and applicant details. That gives us what we need to prepare a personalised quote response, subject to eligibility and underwriting. No documents, and no health information.
The applicationIdentity details for each person to be insured, where you’ll be living in Spain, your policy choices, payment information and the health questionnaire.
Identity

An identity document for each person insured.

Each applicant should have a valid identity document available. As the application stage is being set up, it is intended to accept passport or NIE details, with an upload for each person to be insured.

If you don’t have a NIE yet, that isn’t necessarily a reason to wait: the application is intended to accept passport details where no NIE is available. That is how the application stage is being set up, not a confirmed operational rule — ask us before you assume it applies to your case.

Whatever you use, enter the names exactly as they appear on the document. Documentation is produced from what you submit, and a mismatch between the two is easier to avoid than to correct.

Where You’ll Live

Your address, and your intended residence in Spain.

The application asks for the address you are at now and, separately, the town or city and postcode where you will be living in Spain. Both are asked for — they are not assumed to be the same, and you do not need to have moved to complete an application.

Where you will be living is not an administrative detail. Premiums are calculated using the age attained and the geographical zone, so the location affects the price you are quoted.

Health Information

Nothing medical belongs at quote stage.

You will be asked health questions, and each person to be insured answers them individually. That happens only at the application stage.

You do not need medical records, a doctor’s letter or a diagnosis history to get a quote, and you should not send any of it with one. If your history is complicated, our pre-existing conditions guide covers how declarations are treated, and how to apply covers where in the sequence the questions arrive.

Payment

What the conditions settle about paying.

What payment information you need depends on the method agreed. Three things are settled by the policy conditions; the rest is confirmed with you rather than published here.

1
The premium is annual, and may be divided into biannual payments with a surcharge.Source: Cl. VI.4.3.
2
Payment is by direct debit, or another expressly agreed method. The conditions provide for alternatives without listing them, so the method that applies to you is agreed as part of your application.Source: Cl. VI.4.6.
3
The first premium must be paid before cover takes effect. Cover does not begin at submission or acceptance.Source: Cl. VI.4.1.

We have deliberately not published a list of accepted payment methods. The payment methods available for your application will be confirmed with your personalised quote and application instructions.

Couples And Families

More than one person on an application.

Where more than one person is to be insured, each provides their own identity details and each answers the health questionnaire individually. One person’s answers are not taken to cover anyone else.

One rule the conditions set explicitly: people under 14 are insurable only where their custodians or guardians are also insured. A child cannot be covered alone.

How a couple or family is arranged where circumstances differ between them is not something we set out here — see the open items below. Bring us the situation and we will go through it with you.

After The Policy Starts

One thing is required later.

Once the visa has been issued, evidence of issuance must reach the insurer within two months of the policy start date; otherwise the contract terminates automatically under the stated terms.

It is worth knowing about at the point you apply, because the two-month clock runs from the policy start date rather than from the date the visa arrives.

What you do not need to request a quote.

Worth stating plainly, because gathering these first is wasted effort.

Start with a quote

Ages, dates and where you’re heading is genuinely enough to begin. Everything else belongs to the stage after.

No documents and no medical information are collected at quote stage.

Common Questions

Your questions, answered.

Yes — a quote asks for no identity document at all. And an application can be started using passport details where no NIE is available.
You need to know where you intend to live. The application asks for the address you are at now and, separately, the town or city and postcode you are moving to — so you do not need to have moved, or to hold a Spanish address already.
Each person to be insured provides their own identity details, and the application provides an upload for each of them. Each also answers the health questions individually rather than being covered by someone else’s answers.
Use the passport spelling, exactly as printed, including any accents or their absence. Documentation is produced from what you submit. If your documents already disagree with each other, tell us before you apply rather than after.
No. The health questionnaire is answered at application stage, and it asks questions rather than requesting records. Don’t attach medical documents to a quote request.
The conditions provide for payment by direct debit or another expressly agreed method. Tell us how you would prefer to pay and we will confirm what applies to your application.
